Health Care for the Homeless seeks a Mobile Driver/Outreach Worker to operate and transport the mobile clinic, assist with setup, and engage clients in outreach settings across Baltimore. You will help connect individuals to care, document encounters in HMIS, and support clinic operations with the clinical team.

The role requires a valid driver’s license, a high school diploma or GED, and experience serving vulnerable populations.

Competenze

• High school diploma or GED required; associate degree preferred.
• Valid driver’s license with clean driving record required.
• Experience engaging with vulnerable populations is highly valued.

Mansioni

• Safely operate and transport the mobile clinic and perform basic vehicle checks.
• Assist with mobile clinic setup/breakdown and manage equipment and supplies.
• Engage clients in outreach settings and facilitate referrals to care and services.
• Document outreach encounters and client interactions in HMIS/AthenaOne as required.
• Provide basic client support and linkage to care, including follow-up.
