Qualifications

• Licenses: Texas State Driver's License or CDL; BLS from AHA-approved program.
• Experience: 2+ years operating large commercial vehicles; relevant to Mobile Medical Fleet Outreach.
• Language: Bilingual; Spanish preferred but not required.

Responsibilities

• Conduct in-reach, outreach, street medicine and educational activities for patients and the homeless community.
• Safely drive and operate HCHP mobile units to designated locations and on planned schedules.
• Serve as primary contact for patients in the HCHP population and coordinate with the care team to improve health outcomes.
• Demonstrate professional, friendly interactions with team members and patients.
