Access Healthcare Associates is seeking a dedicated Psychiatric Mental Health Nurse Practitioner (PMHNP) to join our Mobile Geriatric Psychiatry Team in delivering in-home and senior living-based psychiatric care. The role blends independent practice with strong team collaboration, supported by a care model centered on dignity and outcomes for aging populations.

Qualifikationen

Active PMHNP license in the practicing state.
Experience in geriatric psychiatry, long-term care, or mobile healthcare preferred.
Ability to practice autonomously with remote support as needed.

Aufgaben

Conduct comprehensive psychiatric evaluations for geriatric patients in home and facility-based settings.
Diagnose and manage mood disorders, anxiety disorders, dementia-related behaviors, and other psychiatric conditions.
Prescribe and manage psychotropic medications with geriatric considerations.
Provide crisis intervention, behavioral guidance, and family education when indicated.
Collaborate with primary care providers, therapists, and facility teams for coordinated care.
Build trusted relationships with patients, caregivers, families, and community partners.
Maintain accurate clinical documentation and follow-through.
