Job summary

Brigade Health, through Access Healthcare Associates, seeks a dedicated PMHNP to provide geriatric psychiatric services in home and senior living settings. This role blends independent practice with strong team collaboration to deliver compassionate, patient-centered care to seniors.

Qualifications

• Active PMHNP license in the practicing state.
• Experience in geriatric psychiatry or long-term care preferred.
• Ability to work independently with remote support.

Responsibilities

• Conduct comprehensive psychiatric evaluations of geriatric patients in home or facility settings.
• Diagnose and manage mood, anxiety, and dementia-related behavioral symptoms.
• Prescribe and manage psychotropic medications for older adults.
• Provide crisis intervention and family education as needed.
• Collaborate with primary care, therapists, and care teams for coordinated care.
• Maintain thorough clinical documentation and follow-through.
