Job summary

Access Healthcare Associates is seeking a dedicated PMHNP to provide geriatric psychiatric care in home and senior living settings across the region. This role blends independent clinical practice with strong team collaboration, supported by a dedicated medical assistant and medical scribe.

You will develop expertise in dementia-related behaviors, depression, and anxiety, delivering compassionate, patient-centered care and collaborating with multidisciplinary teams to improve outcomes for older

Qualifications

Active PMHNP license in the practicing state.
Experience in geriatric psychiatry preferred.
Ability to work independently with remote support.
Strong communication and compassionate patient care.

Responsibilities

Conduct comprehensive psychiatric evaluations for geriatric patients in home and facility-based settings.
Diagnose and manage mood disorders, anxiety, dementia-related behaviors and other psychiatric conditions.
Prescribe and manage psychotropic medications with geriatric considerations.
Provide crisis intervention, behavioral guidance, and family education when indicated.
Collaborate with primary care, therapists, and care teams for coordinated care.
Build trusted relationships with patients, caregivers, families, and community partners.
Maintain accurate clinical documentation and follow-through.
