Job summary

Access Healthcare Associates, part of Brigade Health, seeks a dedicated PMHNP for a Mobile Geriatric Psychiatry role serving seniors in homes and senior living communities across California.

You will practice independently with strong team collaboration, delivering continuum-based psychiatric care, treating mood/anxiety and dementia-related issues, and coordinating with PCPs and facility teams. A dedicated MA and scribe support your clinical work.

Qualifications

• Active PMHNP license in California.
• Geriatric psychiatry or mobile healthcare experience preferred.
• Ability to work independently with remote support.
• Strong collaboration and communication skills.

Responsibilities

• Conduct comprehensive psychiatric evaluations in home and facility settings.
• Diagnose and manage mood, anxiety, and dementia-related conditions.
• Prescribe and manage psychotropic meds with geriatric considerations.
• Provide crisis intervention and family education when needed.
• Collaborate with primary care, therapists, and care teams for coordinated care.
• Maintain accurate clinical documentation and follow-through.
