Job summary

Access Healthcare Associates, part of Brigade Health, is seeking a dedicated PMHNP to provide mobile geriatric psychiatry services in homes and senior living communities. You will conduct comprehensive evaluations, manage mood and anxiety disorders, and collaborate with caregivers and primary care teams to ensure holistic patient care.

Qualifications

• PMHNP license active in the state you practice
• Experience in geriatric psychiatry, long-term care, or mobile healthcare preferred
• Strong clinical judgment and communication skills

Responsibilities

• Conduct comprehensive psychiatric evaluations for geriatric patients in home and facility-based settings.
• Diagnose and manage mood disorders, anxiety disorders, dementia-related behaviors, and other psychiatric conditions.
• Prescribe and manage psychotropic medications considering comorbidities and geriatric physiology.
• Provide crisis intervention, behavioral guidance, and family education when clinically indicated.
• Collaborate with primary care providers, therapists, and facility care teams for coordinated, whole-person care.
• Build trusted relationships with patients, caregivers, families, and community partners.
• Maintain consistent, accurate clinical documentation and follow-through.
