Job summary

Access Healthcare Associates seeks a PMHNP to deliver geriatric psychiatric services in home and senior living settings. You will diagnose, treat, and support patients with mood and cognitive disorders, while coordinating care with families and care teams.

You’ll practice with autonomy and remote support, focusing on compassionate, relationship-based care that improves outcomes for older adults across our partner communities.

Qualifications

• Active PMHNP license in the practicing state.
• Experience in geriatric psychiatry, long-term care, or mobile healthcare.
• Ability to work independently with remote support.
• Strong clinical judgment and communication skills.
• Compassionate, patient-centered approach with seniors.

Responsibilities

• Conduct comprehensive psychiatric evaluations for geriatric patients in home and facility settings.
• Diagnose and manage mood, anxiety, dementia-related, and other psychiatric conditions.
• Prescribe and manage psychotropic medications with geriatric considerations.
• Provide crisis intervention, behavioral guidance, and family education.
• Collaborate with primary care, therapists, and facility teams for coordinated care.
• Build trusted relationships with patients, caregivers, families, and community partners.
• Maintain accurate clinical documentation and follow-through.
