Job summary

Access Healthcare Associates, part of Brigade Health, is seeking a dedicated PMHNP to deliver geriatric psychiatric care in homes and senior living communities. You will practice independently while collaborating with a dedicated medical assistant and scribe to ensure high-quality, patient-centered care.

Qualifications

• Active, unencumbered PMHNP license in the practicing state.
• Experience in geriatric psychiatry, long-term care, or mobile healthcare preferred.
• Strong clinical judgment and communication skills.

Responsibilities

• Conduct comprehensive psychiatric evaluations for geriatric patients in home and facility settings.
• Diagnose and manage mood disorders, anxiety, dementia-related symptoms, and other psychiatric conditions.
• Prescribe and manage psychotropic medications with consideration of comorbidities and geriatric physiology.
• Provide crisis intervention, behavioral guidance, and family education when indicated.
• Collaborate with primary care providers, therapists, and facility care teams for coordinated care.
• Build trusted relationships with patients, caregivers, families, and community partners.
• Maintain accurate clinical documentation and follow-through.
