The Columbus Organization is seeking a field-based Case Manager to support individuals with intellectual challenges in Georgia. You will coordinate services, gather information for the ISP, and help navigate Medicaid eligibility and community resources.

The role requires travel within the assigned service area, strong communication skills, and the ability to maintain timely documentation and client contacts. A Bachelor’s in human services is required, with experience in DD/IDD settings.

Qualifikationen

• Minimum bachelor’s degree in human service field (social work, psychology, nursing, or related).
• Two years’ experience serving persons with DD/IDD or related disabilities and two years in healthcare, habilitative/rehabilitative settings.

Aufgaben

• Assists people with intellectual challenges or developmental disabilities to access and coordinate needed support in community settings.
• Coordinates gathering information for the support plan and arranges clinically pertinent assessments.
• Reviews information and recommendations for interventions prior to ISP meetings.
• Helps individuals and families identify cost-effective services and supports.
• Assists with Medicaid eligibility applications and maintenance.
• Monitors Medicaid redetermination dates and completes required paperwork.
• Maintains documentation and ensures HIPAA/privacy compliance.
• Maintains monthly face-to-face contact and monitors ISP outcomes.
