Job summary

The Columbus Organization is seeking an Indiana Case Manager to coordinate services for individuals with intellectual and/or developmental disabilities. You will ensure access to waivers, Medicaid supports, and community-based services within a person-centered framework.

Qualifications

• Bachelor's degree in a human services-related field or equivalent experience.
• Experience supporting individuals with intellectual and/or developmental disabilities (IDD).
• Familiarity with DDRS, BQIS, HIPAA, and state regulations is preferred.
• Strong documentation and reporting skills to maintain records and plans.

Responsibilities

• Serve as Primary Liaison with participants, families, guardians, and providers.
• Ensure compliance with HIPAA and state regulations in all records.
• Develop and implement person-centered plans (PCISP).
• Complete annual planning, budget development, and LOSCI determinations.
• Lead IST meetings at least semi-annually with participant involvement.
• Conduct required face-to-face visits, including annual in-home visits.
• Maintain monthly contact and ensure documentation within 7 days.
• Monitor service delivery and utilization against PCISP and supports.
• Promote health, safety, and risk mitigation for participants.
• Document outcomes, satisfaction, and participate in quality improvement.
