Qualifications

• Bachelor's degree in a human service field (social work, psychology, special education, nursing, or related).
• 1+ year experience serving persons with developmental disabilities in case management or coordination; Master’s may substitute.

Responsibilities

• Coordinates information gathering for the support plan and arranges assessments.
• Reviews clinical information to inform ISP meetings and interventions.
• Assists individuals and families in identifying cost-effective services and Medicaid eligibility.
• Monitors Medicaid redeterminations and manages related paperwork.
• Oversees ISP service participation and ensures services meet individual needs.
• Maintains monthly face-to-face contact and documents all contacts in the support notes.
• Ensures timely submission of required documentation and adheres to HIPAA and policy requirements.
