Job summary

The Columbus Organization in Indiana seeks an Indiana Case Manager to coordinate services for individuals with intellectual and/or developmental disabilities (I/DD), ensuring access to waivers, Medicaid State Plan, and community-based supports. This field-based role requires travel within Boone County or Hamilton County and surrounding areas, with remote administrative work when not visiting clients.

Qualifications

Bachelor’s degree in a human services field or equivalent experience.
Ability to travel within Boone/Hamilton counties and surrounding areas.
Strong interpersonal and relationship-building skills.
Commitment to person-centered practices and advocacy.
Valid driver’s license and reliable vehicle.

Responsibilities

Serve as Primary Liaison with waiver participants, families, guardians, providers, and state agencies.
Develop Person-Centered Plans (PCISP) aligned with individual goals.
Conduct annual planning, budget development, and LOCI.
Lead IST meetings at least semi-annually with participant and representatives.
Perform required face-to-face visits, including annual in-home visits.
Maintain monthly contact and document encounters within required timelines.
