Job summary

Santé Group seeks a dedicated Family Peer Support Specialist to join the MRSS team in Cambridge, MD. You will work with a clinician to provide immediate crisis stabilization to youth and caregivers in the community, de-escalate crises, and help ensure continuity of care through collaboration with partners.

Qualifications

• High School Diploma or equivalent required.
• At least 1 year of personal lived experience caring for your own child or a child you have guardianship over.
• 3 years personal lived experience caring for a child with behavior dysregulation or a mental health diagnosis or co-occurring disorder.

Responsibilities

• Partner with a clinician to respond to community-based crisis calls involving youth and caregivers.
• Promote health, self-direction, and skill development for youth and families.
• Support the development of individualized goals focused on long-term stabilization.
• Provide education on available resources, including: Community services, Support groups and networks.
• Serve as an advocate for youth and caregivers, ensuring their voices are heard and respected.
• Assist in wraparound planning and the development of crisis and family-driven service plans.
• Facilitate smooth transitions (“warm hand-offs”) to ongoing services and supports.
• Participate in community stakeholder meetings related to the care of involved youth.
