Responsibilities

• Provide general medical care and care coordination for adult, women’s health, pediatric and geriatric populations.
• Perform histories, physicals, preventive assessments and diagnose medical conditions.
• Order and interpret diagnostic tests; refer to specialists when needed.
• Develop and implement care plans and document medical records in EMR.
• Travel up to ~120 miles daily; participate in community—based clinics and home visits.
