Job summary

Molina Healthcare is seeking a Nurse Practitioner to provide screening, preventive and medical care in home, community and facility settings. You will perform histories, physicals, and care coordination to close gaps in care for diverse patient populations.

In this role you’ll manage chronic and acute conditions, order tests judiciously, and contribute to team-based care under physician supervision. Travel within the region is expected, with occasional overnight stays.

Qualifications

• At least 1 year of experience as a nurse practitioner.
• Active national certification from AANP or ANCC.
• Current state-approved Family Nurse Practitioner license.
• DEA license to prescribe.
• BLS certification.
• Valid driver's license and reliable transportation.
• Ability to work in varied settings and travel.
• Strong communication in all forms.
• Calm under pressure and capable of multi-tasking.
• Proficient with Microsoft Office and EMR systems.

Responsibilities

• Provide screening, preventive and medical care in home, community and facility settings.
• Perform history, physical exams and manage gaps in care.
• Address chronic and acute primary care needs and assess urgency.
• Document diagnoses and coordinate care plans.
• Arrange specialty consults as needed.
• Order diagnostic tests judiciously and manage resource use.
• Develop care plans and schedule follow-ups.
• Conduct face-to-face visits across multiple locations.
• Participate in community-based clinics and education.
• Travel up to 120 miles daily within the region; may require overnights.
