Job summary

Molina Healthcare seeks a Family Nurse Practitioner to provide screening, preventive and primary care in-home, community and nursing facility settings. You will conduct evaluations, coordinate care, order tests, and manage care plans to improve member outcomes.

Ideal candidates hold an active NP certification with FNP license and DEA; BLS; willing to travel; proficient in EMR and MS Office; experience with diverse populations is a plus.

Qualifications

• At least 1 year of experience as a nurse practitioner or equivalent.
• Active and unrestricted national NP certification (AANP/ANCC).
• Current state license to practice as a Family Nurse Practitioner (FNP).
• DEA license with authority to prescribe in state of practice.
• BLS certification Health and safety.
• Valid driver's license and reliable transportation for travel.

Responsibilities

• Provide general medical care and care coordination for diverse member populations.
• Perform comprehensive history and physicals for gaps in care and preventive services.
• Order and interpret diagnostic tests within resource limits.
• Develop and implement a medical plan of care and coordinate with physicians.
• Conduct in-home, community, and facility visits and telehealth as needed.
• Travel up to 120 miles daily to visit sites within assigned region.
