Job summary

Molina Healthcare is seeking a registered Nurse Practitioner to provide screening and primary medical care across home health, community settings, and pop-up clinics. The role involves care coordination, diagnostic testing, and collaboration with physicians to develop patient care plans.

Responsibilities include in-home visits, nursing facility rounds, and remote/alternative modality visits as needed, with regional travel to client sites and adherence to state regulations.

Qualifications

• Active and unrestricted NP certification (AANP/ANCC).
• Current FNP license active and unrestricted in state of practice.
• DEA license to prescribe with authority in state of practice.
• BLS certification valid and current.
• Valid driver's license and reliable transportation for travel requirements.

Responsibilities

• Provide general medical care and care coordination to diverse patient populations.
• Perform history, physical exams and preventive assessments to identify gaps in care.
• Order or perform diagnostic tests and coordinate with physicians as needed.
• Develop and implement a plan of care and schedule follow-up visits.
• Provide post-discharge coordination to reduce readmissions.
• Make timely medical records entries in the EMR system.
