Job summary

Central Counties Services seeks a highly capable clinician to perform complex clinical case work in diverse community settings. You will conduct crisis assessments, determine service eligibility, and coordinate appropriate care for clients in homes, jails, shelters, and hospitals.

The role requires frequent travel, flexible schedules including evenings and weekends, and the ability to work independently with limited supervision while maintaining strict confidentiality and ethical standards.

Qualifications

• Graduation from an accredited four-year college or university with a major in psychology, social work, or a related field.
• Experience in a field relating to the agency program may be required.
• Ability to perform tasks with minimal supervision.

Responsibilities

• Conduct crisis assessments and determine appropriate hospital or community-based services.
• Coordinate with healthcare and law enforcement partners to ensure client safety.
• Document care plans, progress notes, and outcomes in the EHR system.
• Provide on-call coverage and after-hours support as assigned.
• Travel frequently within the service area to meet client needs.
