Job summary

A Bachelor’s degree in social work or related field is required, with at least one year of direct adult or ACT population experience, and completion of mandated case-management

Qualifications

• Bachelor’s degree in social work or related field as approved by Medicaid or funding source.
• At least one year of full-time employment experience with adults or ACT population with direct daily contact in a human service setting.
• Completion of state-required case management/service coordination course within six months of date of employment.

Responsibilities

• Conducts and documents comprehensive assessment of client and/or client’s needs.
• Participates in development of client’s service plan, consistent with assessment.
• Arranges for delivery of needed services, as identified in assessment.
• Assists client in accessing needed services through referrals or linkages.
• Monitors client’s progress by making referrals, tracking appointments, and performing follow-up on services rendered.
• Performs crisis intervention and advocates on behalf of client.
• Maintains case records documenting contacts, services needed, reports, and progress.
