Job summary

Bear River Behavioral Health Services is seeking a full-time Crisis Response/Case Manager in Logan, UT. The role involves mobile crisis intervention in homes or community settings to reduce risk, with subsequent treatment planning and resource linkage.

You will perform CMNA-based planning, coordinate with police and hospitals, and document care accurately in the EMR while attending supervision and training sessions. Requires teamwork and weekend flexibility.

Qualifications

• Bachelor's degree in behavioral science. Ability to obtain SSW licensure, preferred but not required.
• Must have a Utah driver’s license and good driving record.
• Must pass Criminal Background Investigation.
• Ability to work well in a team and may require some weekend work.

Responsibilities

• Provide mobile crisis response to youth/adults in homes or community settings.
• Perform Case Management functions and develop CMSP-based service plans.
• Coordinate crisis services with police, hospitals, and partners; ensure linkage to resources.
• Maintain accurate documentation in the clinical records system and attend trainings.
