Responsibilities

• Develop discharge plans with the multi-disciplinary team to ensure continuity of care.
• Conduct psychosocial assessments and provide crisis and bereavement interventions as needed.
• Educate and coordinate with patient care teams and outside agencies to ensure timely discharge and quality care.
