Job summary

Four Corners Community Behavioral Health is seeking a full-time Case Manager for day shift (6 a.m. – 6 p.m.) at the Crisis Receiving Center in Price, UT. Responsibilities include greeting clients, assisting with intake, enforcing center rules, and ensuring a safe environment.

The role involves crisis de-escalation, connecting clients to community resources, and maintaining a clean workspace. Some lifting (20 lbs) and occasional transportation may be required.

Qualifications

• Strong communication and teamwork abilities required.

Responsibilities

• Greet clients in a crisis setting and assist with intake paperwork.
• Notify clients of center rules and ensure a safe, positive environment.
• Assist with crisis de-escalation and connect clients to community resources.
• Maintain a clean, safe work area and complete daily center tasks.
• Coordinate discharge planning with clients and staff as needed.
