Qualifications

• Must have a valid driver’s license.
• Must maintain automobile liability insurance as required.
• Willing to complete Case Management training upon hire.

Responsibilities

• Coordinate case management in the community via Mobile Crisis Team.
• Develop and implement case management service plans and follow up with clients.
• Monitor progress and provide follow up services as needed.
• Travel within catchment areas to provide services and support.
