Job summary

Wellstone is seeking Experienced Part-Time Case Managers for the Mobile Crisis Team in Huntsville, AL. Shifts are 7AM-7PM or 7PM-7AM on Saturdays and Sundays, with up to 80% travel.

You will provide outpatient case management and direct services to seriously mentally ill clients, coordinating care with local responders and therapeutic teams. The role requires a Bachelor’s degree in a related field, a valid 3-year driving record, and strong communication and organizational skills.

Qualifications

• Bachelor’s degree or higher in a human services related field as indicated
• 3-year driving record and reliable transportation required
• Excellent verbal and written communication skills required
• Successful completion of Alabama Case Management Training required
• Knowledge of local community resources and agencies essential
• Personal automobile liability insurance in acceptable amounts

Responsibilities

• Provide outpatient case management services to seriously mentally ill population
• Provide direct services to address needs and continuum of care information
• Provide money management training related to psychiatric stability
• Follow-up progress with treatment teams
• Evaluate client’s residential stability and assist in obtaining stable environment
• Provide Basic Living Skills training where needed
• Assist clients with obtaining medications and monitor adherence
• Provide collateral services such as transportation and referrals
• Maintain high quality client documentation and electronic charting
• Develop Individual Service Plans and review progress every 3-12 months
• Collaborate with Residential Care managers to meet client needs
• Perform incidental care to families of mentally ill clients
• Complete other duties as assigned
