Job summary

Volunteers of America Chesapeake & Carolinas seeks a PRP Community Living Counselor to deliver community support services based on each consumer’s plan. You will serve primarily in the community and natural settings, helping clients achieve rehabilitation goals and maintain stability.

Responsibilities include developing individualized service plans, coordinating referrals, and maintaining accurate documentation while collaborating with the treatment team and ensuring timely progress notes.

Qualifications

• Experience in human services settings; ability to work in community-based multicultural environments.
• Must be able to develop effective working relationships with consumers.
• Ability to work independently with limited supervision and multitask in a non-traditional environment.
• Strong oral and written communication skills; proficient with standard office software.

Responsibilities

• Manage a caseload of approximately 40 adults.
• Assist consumers in developing interpersonal and community coping skills.
• Assist in relapse and symptom management planning.
• Participate in Individualized Service Plan development with measurable objectives.
• Provide and coordinate referrals for consumer needs.
• Conduct visits to consumer job sites, homes, and other settings.
• Transport consumers as needed; arrange transportation if necessary.
• Adhere to documentation requirements and complete billable activities.
• Meet weekly with Treatment Team to discuss issues.
• Submit weekly billing and progress notes.
• Maintain community resources and referral index.
• Provide crisis intervention when necessary.
• Meet weekly/monthly productivity requirements (400 units per month).
