Job summary

CareLink Community Support Services seeks compassionate team members to assist individuals with disabilities in daily living, recovery, and independence. You will implement person-centered activities, promote community integration, and document progress in a respectful, supportive manner aligned with the agency's mission.

You will help residents with daily living tasks, meals, and medication management, while maintaining a safe, welcoming environment and fostering self-determination and dignity.

Qualifications

• High School Diploma or GED required.
• Bachelor's degree preferred.
• Field of study: Human Services or related field.
• 1–2 years of work experience in human services preferred.

Responsibilities

• Develop and implement service/recovery plans aligned with resident preferences and goals.
• Assess, teach, evaluate daily living skills and related activities.
• Complete daily written documentation reflecting recovery progress and interventions.
• Assist with medication regimen and PRN medications.
• Support engagement in therapeutic activities and community participation.
• Encourage positive relationships with family and others.
• Maintain cleanliness and order in living areas and rooms.
• Prepare meals and accommodate dietary considerations as required.
• Access community medical resources and assist with transportation needs.
• Respond to safety/emergency situations and drive agency vehicles when needed.
