Job summary

Volunteers of America Chesapeake & Carolinas is seeking a PRP Community Living Counselor to conduct community support activities and help consumers achieve rehabilitation goals defined in their ISPs. About 90% of duties occur in the community or consumers’ natural settings.

The role emphasizes outreach, documentation per MHRS/VOAC standards, and collaboration with the treatment team to ensure consumer well-being and progress.

Qualifications

• High School or GED required with 2 years of college or 3 years of relevant human services experience.
• At least 1 year of related human service activities/experience.
• Experience in community-based and multicultural settings preferred.
• Ability to develop working relationships with consumer population.
• Self-starting and ability to work independently with limited supervision.
• Motivated and able to motivate others.
• Able to relate to individuals making life decisions.
• Ability to multitask in a non-traditional work environment.
• Strong organizational and oral/written communication skills.
• Experience with MS Word/Excel/PowerPoint/Outlook and EMR systems.
• Valid driver’s license; CPR/First Aid certification required.
• Bilingual abilities are a plus.

Responsibilities

• Manage a caseload of approximately 40 adults.
• Assist consumers in developing interpersonal and community coping skills.
• Assist in relapse and symptom management planning.
• Participate in ISP development with specific, measurable objectives.
• Provide and coordinate referrals for consumer needs.
• Conduct visits to job sites, homes, hospitals, and community settings.
• Transport consumers in a personal vehicle as needed.
• Turn in weekly billing/progress notes; meet weekly with Treatment Team.
• Maintain a community resources and referral index.
• Provide crisis intervention when necessary.
• Meet weekly/monthly productivity requirements (400 units per month).
