Job summary

CareLink Community Support Services is seeking a dedicated role supporting individuals with disabilities to live independently in the community. You will implement person-centered activities, document progress, and promote recovery and wellness in a supportive environment.

The position requires assisting with daily living, meals, housekeeping, and community activities while maintaining safety and dignity for program participants.

Qualifications

• High School Diploma or GED is required.
• Bachelor's degree is preferred.
• Valid Driver’s License is required.
• CPRP/RPRP Certification is preferred.
• Certified Peer Specialist is preferred.
• Experience in Human Services field or relevant life experience is preferred.

Responsibilities

• Develop and implement service/recovery plans reflecting the individual’s preferences, goals, lifestyles, and interests.
• Assess, teach, evaluate, and supervise daily living skills, meal planning, housekeeping, career needs, and social, recreational, and community activities.
• Complete written documentation daily reflecting recovery progress and interventions.
• Assist and oversee individuals with medication regimen including PRN medications.
• Engage in productive/therapeutic activities and support positive relationships with family and others.
• Assist individuals with daily living tasks, personal hygiene, and home maintenance.
