Job summary
Your Health in Augusta is seeking a dedicated Community Health Worker to service patients in the Augusta area. The CHW bridges patients with healthcare services and community resources, including home visits and ALFs, to ensure timely access to care and essential supplies.
This full-time role requires reliable transportation and daily travel, with 12-hour shifts (7:00 AM–7:00 PM). Responsibilities include delivering medications and DME, coordinating telehealth with providers, and supporting care
Qualifications
- Certified Medical Assistant, Paramedic, or Licensed Nurse in good standing.
- Phlebotomy experience preferred; willing to train.
- Experience with DME delivery or logistics preferred.
- Must have reliable transportation and a valid driver's license.
Responsibilities
- Work with care team to coordinate patient care and follow the care plan.
- Deliver medications and review instructions during home visits.
- Deliver DME and provide training on use in home/facility settings.
- Coordinate telehealth visits with pharmacists, specialists, and PCPs.
- Perform venipuncture for labs and collect specimens when needed.
- Utilize remote patient monitoring and EMR tools in daily work.
