Job summary
Your Health Organization is seeking a dedicated Community Health Worker (CHW) to support patients across Bogart, Winder, Loganville and surrounding areas. The CHW will visit patients at homes and ALFs, deliver medications and DME, and coordinate with care teams to improve access to care and service delivery.
Primary duties include ensuring seamless care, coordinating telehealth visits, and providing RPM training and support.
Qualifications
- Certified Medical Assistant, Paramedic, or licensed nurse preferred.
- Phlebotomy experience preferred; willing to train if needed.
- Public health or home health experience preferred.
- Experience with DME delivery or logistics preferred.
- Proficient with RPM, telemedicine, and EMR tools.
Responsibilities
- Coordinate patient care with the care team to align with the care plan.
- Communicate with patients via phone, email, Teams, or in person.
- Coordinate home and ALF visits with PT/OT and providers to support care.
- Deliver medications to patients' homes; review instructions and reconcile meds.
- Facilitate telehealth visits with pharmacists, specialists, and providers.
- Set up RPM equipment training and ongoing support for patients and caregivers.
- Deliver DME to homes and ALFs; ensure proper use and training.
- Perform routine venipuncture for lab testing and handle specimens.
- Assist with scheduling and mobility as needed; travel between offices and facilities.
