Job summary

Your Health is seeking a dedicated Community Health Worker to serve patients in the Charleston area, performing home and ALF visits, delivering medications and DME, and coordinating care with providers. The role requires reliable transportation and travel daily within the service area.

The CHW will coordinate visits with care teams, support telehealth sessions, and assist in RPM setup and patient education. Strong communication and multitasking skills are essential for success.

Qualifications

• Certified Medical Assistant, Paramedic, or Licensed Nurse preferred with good standing license.
• Phlebotomy experience preferred; willing to train if not experienced.
• Public health or home health experience preferred.
• Experience with DME delivery or logistics is a plus.
• Proficiency with technology tools for RPM, telemedicine, and EMRs.

Responsibilities

• Visit patients in homes and ALFs to coordinate care with providers.
• Deliver medications and DME, confirm instructions, and reconcile medications.
• Coordinate telehealth visits between patients and pharmacists, specialists, and PCPs.
• Set up RPM equipment training and provide ongoing support to patients and caregivers.
• Perform routine venipuncture and collect samples during visits as needed.
• Travel between offices, facilities, and patient homes with reliable transportation.
