Job summary

Your Health Organization in the United States is seeking a dedicated Community Health Worker to serve patients in Bogart, Winder, Loganville, and surrounding areas. The CHW will visit homes and ALFs, deliver medications and DME, and coordinate care with providers to ensure timely access to services.

This is a full-time, salary-based position with 12-hour shifts (7:00 AM–7:00 PM) requiring reliable transportation and daily travel.

Qualifications

• Certified Medical Assistant, Paramedic, or Licensed Nurse preferred; licenses in good standing.
• Phlebotomy experience preferred; willing to train if not experienced.
• Public health or home health experience preferred.
• Experience with DME delivery or logistics operations preferred.
• Proficient with technology tools for remote patient monitoring, telemedicine, and EMRs.
• Strong organizational and time management skills.
• Ability to read and communicate effectively; strong written and verbal skills.
• Must be a licensed driver with insured vehicle and clean driving record.

Responsibilities

• Work with care team to ensure coordinated patient care.
• Communicate effectively via phone, email, Teams, Athena, in-person.
• Coordinate visits with PTs, OTs and Providers in ALFs and homes.
• Deliver medications to patients' homes and review instructions; reconcile lists.
• Coordinate telehealth visits between patients and pharmacists, specialists, providers.
• Set up RPM training and provide ongoing support and troubleshooting.
• Deliver DME to patients in homes and ALFs and provide training on use.
• Perform venipuncture for laboratory testing and proper specimen handling.
• Collect blood specimens during home or facility visits.
