Job summary

Your Health is seeking a dedicated Community Health Worker to service patients in the designated area. The role involves home and facility visits, coordinating with care teams, and delivering medications and DME to ensure timely access to essential supplies.

Candidates should have CMA/Paramedic/Nurse licensure in good standing, with experience in public health or home health preferred. Travel is required daily, and shifts are 12 hours long across early to late hours.

Qualifications

• Licensed healthcare professional with current credentials in good standing.
• Phlebotomy experience preferred; training available if needed.
• Experience in public health or home health settings preferred.
• Familiarity with EMR, telemedicine, and RPM tools.

Responsibilities

• Visit patients in homes and ALFs to coordinate care and visits with providers.
• Deliver medications and DME, ensuring safe handling and timely arrival.
• Coordinate telehealth visits between patients and providers; support RPM setup.
• Perform venipunctures and collect specimens when needed.
• Travel daily to service area; maintain reliable transportation.
