Job summary

Qualifications

• Certified Medical Assistant, Paramedic, or Licensed Nurse preferred; licenses must be in good standing.
• Phlebotomy experience preferred; willingness to train if not experienced.
• Experience in public health or home health preferred.
• Experience with DME delivery or logistics operations preferred.
• Proficient with technology for remote patient monitoring, telemedicine, and EMR.

Responsibilities

• Collaborate with care team to ensure coordinated patient care.
• Coordinate visits with PTs, OTs, and providers in homes and ALFs.
• Deliver medications to patients' homes and reconcile instructions.
• Coordinate telehealth visits with pharmacists, specialists, therapists, and PCPs.
• Set up RPM training and provide ongoing support and troubleshooting.
• Deliver DME to homes and facilities and train patients on use.
• Perform routine venipuncture and collect blood samples during visits.
