Job summary

Your Health Organization is seeking a dedicated Community Health Worker to service patients in the Charleston area, performing home and facility visits and facilitating access to care. The CHW will deliver medications and DME, coordinate telehealth with providers, and support RPM equipment use to ensure timely, safe patient care.

This full-time role requires reliable transportation for daily travel, 12-hour shifts (7:00 AM–7:00 PM), and close collaboration with care teams across clinics, homes,

Qualifications

• Certified Medical Assistant, Paramedic, or Licensed Nurse preferred; licenses in good standing.
• Phlebotomy experience preferred or willingness to complete training.
• Public health or home health experience preferred.
• DME delivery or logistics experience preferred.
• Proficient with technology tools for telemedicine and EMR.
• Ability to read and communicate effectively; strong written and verbal skills.

Responsibilities

• Work with care team to ensure coordinated patient care per care plan.
• Communicate effectively via phone, email, Teams, and in-person.
• Coordinate visits with providers in ALFs and patients' homes.
• Deliver medications to patients' homes, review instructions, reconcile meds.
• Coordinate telehealth visits between patients, pharmacists, and providers.
• Instruct patients on use of RPM equipment and provide ongoing support.
