Job summary

Insight Global in Dayton, OH is seeking a Community Health Worker to partner with an interdisciplinary care team, helping members access healthcare and community resources in homes, clinics, and community settings. You will support care plans and coordinate transportation, reminders, and services while documenting activities per Care Management guidelines.

Qualifications

• Experience in community health work or care coordination.
• Familiar with Medicaid, long-term care, and social determinants of health.
• Ability to navigate health and community resources.

Responsibilities

• Engage with members in homes, hospitals, provider offices, and community settings.
• Build supportive relationships and accompany members to appointments.
• Coordinate transportation, reminders, and service logistics for care plans.
• Document information accurately within Care Management guidelines.
• Collaborate with Case Managers and ICT to improve member experience.
