Job summary

AltaPointe Health Systems seeks field-based outreach staff to partner with Mobile Police in the Homeless Outreach Team, delivering relationship-based engagement to chronically unhoused individuals. You will collect data, coordinate access to housing, benefits, and healthcare, and follow up on resolutions across multiple agencies.

The role emphasizes compassion, crisis de-escalation, and durable connections to services, with continuous collaboration among City departments and local providers.

Qualifications

• Bachelor’s degree in social work, psychology, counseling, human services, or a related field.
• Must be 23 years of age, have a valid Alabama driver’s license and an acceptable driving record.
• Proof of current liability insurance is required.

Responsibilities

• Field Outreach & Relationship Building: ride with an assigned Mobile Police officer, engage chronically unsheltered individuals, and maintain case engagement history.
• Encampment Engagement & Resolution: identify encampments, connect residents to services, and pursue housing placement.
• City & Community Partnership: collaborate with City departments and partner agencies, maintain professional public engagement.
• Documentation & Program Accountability: keep confidential HMIS records and support program reporting.
• Supervision and Record Keeping: participate in case staffing and required trainings.
