Job summary

PATH is seeking an Outreach Case Manager to support clients experiencing homelessness in the Downtown Center BID and Harbor UCLA offices. The role focuses on helping clients access housing, healthcare, and community resources through individualized plans and coordinated services.

You will conduct intake, assess needs, and coordinate housing goals with a trauma‑informed, client‑centered approach. This on‑site position requires driving and a commitment to ending homelessness in California.

Qualifications

• Analytical skills and attention to detail to inform decision-making.
• Collaboration and teamwork with colleagues, participants and stakeholders.
• Clear and effective written and verbal communication.
• Creativity and initiative with proactive problem solving.
• Critical thinking to analyze information and plan outcomes.
• Maintain ethical boundaries and professional standards.
• Two years of experience in healthcare, case management, homeless services or a related field.
• CPR/First Aid training and eligibility to work in the U.S.

Responsibilities

• Conduct intake and develop ISP addressing housing goals and income.
• Provide referrals, linkages, information, and support to resources.
• Complete program assessments (e.g., VI‑SPDAT).
• Evaluate strengths and challenges with 90‑day reviews.
• Maintain documentation per PATH policies and HIPAA.
• Develop therapeutic relationships to support housing goals.
• Provide crisis intervention and risk assessment services.
• Use motivational interviewing and trauma‑informed care.
• Maintain knowledge of community resources for wrap‑around services.
