Moms In Motion is seeking a Service Facilitator (SF) Case Manager who will visit clients in their Virginia homes, guiding families through Medicaid Waiver programs and helping ensure safe, healthy, independent living. The role emphasizes compassion, organization, and community focus.

No degree is required; at least 2 years of experience with disabilities or elderly care, and strong communication skills are essential.

Qualifications

• 2+ years supporting individuals with disabilities or the elderly.
• No degree required; heart and experience matter.
• Proficient with email, docs, spreadsheets, portals and video calls.
• Valid driver’s license and reliable vehicle.
• Excellent written and spoken communication.
• Able to pass a background check and provide 2 professional references.
• Bonus points for Person-Centered Thinking/Planning training.

Responsibilities

• Hit the road locally to visit clients in their homes.
• Be the go-to guide for families navigating waiver programs.
• Write up plans of care and assessments that actually make a difference.
• Troubleshoot challenges like service authorizations, timesheets, and more.
• Build lasting relationships with families built on respect, patience, and trust.
• Work remotely from your laptop/tablet.
