Job summary

Moms In Motion / At Home Your Way is seeking a compassionate Service Facilitator (SF) Case Manager to support Virginia families in navigating Medicaid Waiver programs (CCC+, CL & EPSDT). You’ll visit clients at home, helping them stay safe, healthy, and independent while building trusted relationships.

Minimum 2 years of experience assisting individuals with disabilities or the elderly, no degree required, and valid driver's license with reliable vehicle.

Qualifications

• Minimum 2 years of experience supporting individuals with disabilities or the elderly.
• No degree required (we care more about heart and experience).
• Tech-savvy enough for email, docs, spreadsheets, portal navigation and video calls.
• A valid driver’s license and reliable vehicle.
• Excellent written and spoken communication.
• Pass a background check + two professional references.
• Bonus points if you have Person-Centered Thinking/Planning training.

Responsibilities

• Visit clients in their homes for assessments and plans of care.
• Guide families through waiver programs and service authorizations.
• Write care plans and assessments that make a difference.
• Troubleshoot timesheets and other administrative tasks.
• Build relationships with families based on respect, patience, and trust.
• Work remotely from a laptop/tablet when not in the field.
