CareSource Commonwealth Care Alliance is seeking a Care Coordinator to support enrollees in navigating health care, coordinating services, and ensuring timely delivery of care. You will engage with members through community visits, calls, and virtual visits, documenting activities and supporting administrative tasks to improve satisfaction.

Responsibilities include outreach, scheduling coordination, collaboration with care teams, and delivering enrollee-centered care plans.

Qualificações

• High School diploma or GED required.
• Two years healthcare or related experience required.
• Strong communication and problem-solving skills.

Responsabilidades

• Serve as first point of contact for enrollees and assist with plan processes.
• Conduct routine outreach and document care coordination activities.
• Provide in-person visits to enrollees at least annually with consent.
• Maintain documentation and coordinate scheduling across systems.
• Identify social needs and support services for enrollees.
• Lead interdisciplinary care team to create enrollee-centered plans.
• Educate on benefits and service options and address concerns promptly.
• Collaborate with care managers and providers to ensure continuity.
