CareSource/Commonwealth Care Alliance is seeking a Care Coordinator - Massachusetts to serve as the primary contact for enrollees, guiding them through health plan processes and care coordination. The role includes routine outreach, community visits, telephonic interactions, and documentation of activities to support member satisfaction.

Formación

• High School or GED required.
• Two (2) years of experience in healthcare, customer service or a related industry required.

Responsabilidades

• Serve as the first point of contact for enrollees and assist with understanding plan processes.
• Conduct routine outreach to members and providers to confirm service details and coordinate services.
• Provide in-person visits to enrollees at least annually with consent.
• Maintain documentation to ensure visibility across teams and continuity of service.
• Coordinate scheduling, support high-volume outreach, and ensure task completion across systems.
