Job summary

CareSource Commonwealth Care Alliance seeks a Care Coordinator to serve as the primary contact for a stratified panel of enrollees. You will assist with navigating health plans, coordinating services, and documenting activities to ensure timely care delivery and enrollee satisfaction.

Qualifications

• High School or GED required.
• Two (2) years of experience in healthcare, customer service or a related industry required.

Responsibilities

• Serve as the first point of contact for enrollees and assist with understanding health plan processes, appeals and grievances and removing barriers to needed services.
• Conduct routine outreach to members and providers to confirm service details and coordinate services with care teams.
• Provide in-person visits to enrollees at least annually with member consent.
• Maintain open lines of communication with enrollees based on preferences.
• Maintain accurate and timely documentation in internal systems for visibility across teams.
• Coordinate provider and member scheduling; support high-volume outreach and cross-system task completion.
• Identify potential social support or community-based needs.
• Lead the interdisciplinary care team to create enrollee-centered care plans.
• Provide education on plan benefits and service options.
• Escalate concerns and coordinate responses with providers and members.
• Collaborate with care managers and external partners to ensure continuity of care.
• Gather and document information to support care planning and referrals.
• Maintain centralized enrollee records aligned with regulatory standards.
