Job summary

This mobile position requires regular travel to member and provider sites in Ohio, with a focus on timely, compassionate, and effective care planning and execution.

Qualifications

• Associate’s Degree or equivalent years of relevant work experience required.
• Minimum of one year of clinical experience in nursing, social services, or healthcare field required.
• Medicaid/Medicare managed care experience preferred.

Responsibilities

• Coordinate care across settings as part of an interdisciplinary team.
• Travel to member visits, provider visits, and community-based visits as needed.
• Educate members about treatment options and available resources to support informed decisions.
