Job summary

The Columbus Organization is seeking a Full Time Care Coordination professional for Augusta, GA. This field-based role requires travel across defined Georgia counties and remote work when not conducting client visits.

Ideal candidates hold a bachelor’s in a human services field and have 2+ years' experience serving individuals with disabilities within healthcare or rehabilitative settings. Strong coordination, communication, and data skills are essential.

Qualifications

• Bachelor's degree in human services or closely related field.
• At least 2 years' experience serving persons with DD/IDD and 2 years in healthcare/rehabilitative settings.
• Mobile worker; willing to travel within assigned territory; reside in territory.
• Evening and weekend availability as required.

Responsibilities

• Assists people with intellectual challenges or developmental disabilities to access and coordinate needed support and services.
• Coordinates and facilitates all activities related to gathering information for the support plan.
• Arranges and obtains clinically pertinent assessments.
• Reviews clinical information and recommendations for interventions and services prior to ISP meetings.
• Monitors Medicaid eligibility and documents appropriately.
• Maintains charts and documents, with regular monthly contact per DBHDD guidelines.
• Ensures ISP outcomes are met and coordinates service delivery with providers.
