Job summary

CareSource is seeking a Care Guide to coordinate care within an interdisciplinary team, serving as a primary contact for members who require short-term care coordination. The role supports transitions and educates stakeholders about options and benefits.

The position requires a relevant degree or equivalent experience, plus strong communication and problem-solving skills. Travel to member and provider sites is essential to ensure timely and effective care delivery.

Qualifications

• Associate’s Degree or equivalent years of relevant work experience is required.
• Active, unrestricted allied health certification or LPN license may be considered in lieu of Associate’s Degree.
• Medicaid and/or Medicare managed care experience is preferred.

Responsibilities

• Coordinate care as part of the inter-disciplinary Care Coordination Team.
• Serve as single point of contact for care coordination when CME involvement is identified.
• Identify needs, manage barriers to care, and close gaps in care with the Care Manager.
• Document activities and patient responses in line with CareSource policies.
• Travel to member sites and providers to conduct visits as needed.
