Job summary

ChenMed, a physician-led and mission-driven primary care organization, seeks a Primary Care Physician (PCP) to lead comprehensive care for up to 450 patients and partner with a care team to deliver high-quality, cost-conscious care in a value-based model.

The PCP will coach patients, coordinate with hospitalists and specialists, and drive outcomes while aligning with market goals and ChenMed’s culture of love, accountability and passion.

Qualifications

• MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty.
• Must obtain/maintain State Medical License for working state(s).
• Board certification preferred; Board Eligibility required.
• Current DEA number for controlled substances.
• BLS certification within first 90 days of employment.
• Fluent English, verbal and written.

Responsibilities

• Provide care for older adult patients with acute and chronic illnesses.
• Take accountability for patient care outcomes and seek specialist consultation when needed.
• Collaborate with hospitalists when patients are in hospital.
• Lead care team including medical assistant, care facilitator, and care coordinator.
• Coordinate care for patients in hospital, SNF, LTC or homebound settings.
• Cover for other providers and assist in recruiting patients and additional providers.
• Supervise APPs and remain accountable for their actions.
• Perform other duties as assigned by manager.
