Job summary

ChenMed, a physician-led, mission-driven primary care organization, is seeking a Lead, Primary Care Physician to join our growing clinical leadership team. The Lead PCP will oversee a panel of at least 360 patients, drive high-quality outcomes, coach care teams, and model our love‑and‑care culture while expanding access to compassionate VIP care for seniors.

You will lead weekly meetings, collaborate with center and market leadership, and advance risk and quality programs.

Qualifications

• Evidence of leadership and clinical governance skills.
• Ability to coach health teams and drive outcomes.
• Strong communication and collaboration with hospitalists and care teams.

Responsibilities

• Independently provides care for patients with acute and chronic illnesses encountered in the older adult patient.
• Takes full accountability for patient care and outcomes and appropriately seeks consultation from specialists when needed.
• Engages with the hospitalist whenever one of their patients is in the hospital.
• Responsible for assessment, diagnosis, treatment, management, education, health promotion and care coordination and documentation for patients with acute and complex chronic health needs.
• Leads their care team consisting of care promoter, care facilitator, and care coordinator for patients able to come to the office.
• For patients unable to come to the office—in hospital, SNF, LTC or homebound, engages with the transitional care team and others.
• Leads supervision meetings and supports center leadership, filling in as needed for clinical leadership duties.
• Performs other duties as assigned and modified at manager’s discretion.
• Available and accessible for patients to build trust and ensure timely care.
• Evidence-based medicine remains central while considering lifestyle and nutrition factors.
• Physician leadership and wellness are emphasized for sustainable practice.
• Quality and patient outcomes are continuously pursued.
