ChenMed LLC is seeking a Primary Care Physician (PCP) to provide high-quality care for an adult patient panel with accountability for outcomes in a value-based system. You will lead a care team and coordinate with hospitalists to manage acute and chronic illnesses while promoting evidence-based practice.

You will work toward center goals, participate in risk and quality programs, and travel as needed within the market to support patient care and recruiting efforts. Fluency in English is required.

Competenze

• MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required.
• Must obtain a State Medical License for the state(s) of work.
• Board certification is preferred; board eligibility required where applicable.

Mansioni

• The PCP independently provides care for patients with acute and chronic illnesses and takes full accountability for outcomes.
• Leads the care team and coordinates care across medical assistants, care facilitators, and coordinators.
• Engages with hospitalists for inpatient care and participates in risk and quality management programs.
• Drives patient health goals, behavioral change, and high-quality outcomes with the center and market leadership.
